A new report from the Department of Agriculture says last summer's Blue Rhino propane plant explosion was caused by human error.
Thousands of propane cannisters erupted for hours back in July 2013.
Residents living near the plant say the sky looked like a giant fireball, with tanks flying into the air and landing in yards.
According to investigators, workers were trying to vent gas vapors from several propane canisters, but did not do this properly. When another employee started up the forklift, it ignited the vapors accumulating in the area, which stated the fire.
Four people were hurt in the explosion, which caused millions of dollars in property and content damage.
